I confirm that I hold an active California medical license and understand it is required to participate in hands-on injecting.*I understand classes are limited to 6 students and enrollment is first-come, first-served. *I understand my seat is not secured until tuition payment is completed. *I acknowledge that tuition payments are non-refundable, except where otherwise required by applicable law. Requests made more than 21 days before the scheduled course date may be transferred to another available Injector Academy training date, subject to availability. Within 21 days of the scheduled course date, tuition is non-refundable and non-transferable.*
