Tear Trough Filler: Precision & Patient Selection

Key anatomical considerations and technique refinements for the delicate under-eye area — one of the most rewarding yet technically demanding regions in aesthetic medicine.

The tear trough is one of the most challenging yet rewarding areas to treat in aesthetic medicine. When done correctly, tear trough filler can transform a patient's appearance — reducing the tired, hollow look that so many patients complain about. But this delicate area demands precision, deep anatomical knowledge, and careful patient selection to achieve safe, natural-looking results.

Understanding Tear Trough Anatomy

The tear trough is a complex anatomical region extending from the medial canthus along the infraorbital rim. Key structures every injector must understand include:

  • Orbicularis Retaining Ligament: This ligament anchors the orbicularis oculi muscle to the orbital rim and creates the distinct depression characteristic of tear trough deformity.

  • Angular Artery and Vein: These vessels course along the nasojugal fold and must be identified and avoided. The angular artery forms an anastomosis between the facial and ophthalmic arterial systems, making intra-arterial injection a genuine risk.

  • Infraorbital Fat Pads: Volume loss and descent of the superficial and deep fat compartments contribute to the appearance of hollowness beneath the eyes.

  • Thin Skin: The infraorbital skin is the thinnest on the body, making irregularities and product visibility more apparent than in other facial regions.

Patient Selection: Who Is a Good Candidate?

Not every patient with dark circles or under-eye hollows is appropriate for tear trough filler. Proper patient selection is the single most important factor in achieving excellent outcomes:

  • Ideal Candidates: Patients with true tear trough hollowing due to volume loss, good skin elasticity, and realistic expectations about what filler can achieve.

  • Poor Candidates: Patients with significant skin laxity, festoons or malar mounds, active periocular infection, unrealistic expectations, or whose under-eye concerns are primarily due to hyperpigmentation rather than volume loss.

  • The Snap Test: Gently pull the lower eyelid away from the globe and release. If the lid snaps back immediately, skin elasticity is adequate. Slow return suggests poor elasticity and higher risk of post-treatment irregularities.

Needle vs. Cannula: Making the Right Choice

The debate between needle and cannula for tear trough injections continues, and both have valid roles:

Needle Technique

  • Greater precision for small aliquots

  • Precise placement at the periosteum

  • Higher risk of vascular injury

Cannula Technique

  • Reduced vascular complication risk

  • Single entry point for broader coverage

  • Less bruising and post-treatment edema

Product Selection

For the tear trough, product selection is critical. Low G' (elastic modulus) hyaluronic acid fillers with minimal hydrophilic properties and small particle size are preferred. Products with higher cohesivity and lower water attraction reduce the risk of the Tyndall effect and late-onset swelling. Always use products specifically indicated or commonly used for this delicate region.

Injection Technique: Step by Step

  1. Mark the patient in an upright position. The tear trough deformity changes significantly between upright and supine positions. Mark boundaries and entry points while the patient is sitting upright with neutral gaze.
  2. Prep and anesthetize. Topical anesthetic and ice are typically sufficient. Avoid excessive local infiltration which can distort anatomy.
  3. Place product deep, at or just above periosteum. Superficial placement is the most common cause of visible lumps and Tyndall effect. Stay deep along the infraorbital rim, lateral to the mid-pupillary line.
  4. Inject small aliquots. 0.05-0.1 mL per injection point, using micro-boluses rather than threading. Less is more — you can always add product at follow-up.
  5. Massage gently. After injection, gentle molding helps distribute product evenly. Avoid aggressive massage that can displace product or increase swelling.
  6. Reassess in seated position. Always have the patient sit up and check for symmetry before they leave.

Complication Prevention & Management

Safety Alert

The tear trough is a high-risk area for vascular complications. Intra-arterial injection into the angular artery can lead to retrograde embolism and vision loss. Every injector must know the signs of vascular occlusion and have an emergency protocol in place — including hyaluronidase — before treating this area.

  • Aspirate before every injection and wait a full 5-10 seconds

  • Know your vascular anatomy — the angular artery location varies between patients

  • Have hyaluronidase ready and know your dosing protocol for vascular emergencies

  • Document everything including product, volume, lot number, and injection technique used
